VST 2.x/3.x Adapter

Fixed stupid 0.7 regression with missing configuration popup for simple VST effects without their own configuration panels.
Removed unintended minimize & maximize buttons in configuration popups.
Internal bug fixes.
Disabled channel offset feature - use some other DSP to fix channel layout before/after VST. If no sane solution exists, I will make a separate DSP for this later.

Supported systems: Windows 32-bit, Windows 64-bit.

Works with foobar2000 v1.6 and newer
